Eligible Accommodations

Accommodations may be provided for, but are not limited to, the following conditions:

  • Deaf or Hearing Impairment
  • Visual Impairment
  • Learning Disabilities
  • ADD or ADHD
  • Traumatic Brain Injury
  • Psychological Disorders
  • Health Conditions or Mobility Impairments

Documentation Requirements

To receive accommodations, students must provide recently written documentation (within one year) from a licensed clinical professional familiar with the nature and extent of the disability. The documentation must include recommended reasonable accommodations to support the student in an academic environment.

Steps to Receive Academic Accommodations

  1. Student: Complete the Required Forms (available on the Disability Support Services webpage):
    • Application for Disability Support Services (PDF)
    • Letter of Accommodation Request Form (PDF) (Returning students must submit this form each semester to continue accommodations.)
    • Release and Statement Form – Disability Support Services (PDF)
  2. Licensed Professional Submission
    • The licensed professional must complete the Impairment and Disability Assessment (PDF) and submit it to the Student Success Specialist, along with a signed letter on official letterhead outlining recommended accommodations.
  3. Meeting with Disability Services
    • Once all forms are submitted, a meeting will be scheduled to discuss reasonable accommodations. All required paperwork must be submitted before this meeting can take place.
  4. Faculty Notification and Implementation
    • A memo listing the approved accommodations will be sent to the student’s faculty members.
    • A follow-up meeting will be scheduled with the student and faculty members to discuss the implementation of accommodations.

Important Notes

  • Individualized Education Programs/Plans (IEPs) are not accepted. Students must complete the college’s required paperwork.
  • Accommodations must be renewed each semester by submitting a new Letter of Accommodation Request Form.
